Sublimate your patch first: 390 degrees for 0:50 - 1:15 minutes. Pressure - heavy. 
 
ATTACHING SUBLIMATED PATCH TO FABRIC:
 
HEAT PRESS: Let the patch rest for at least 1 hour after it's been sublimated. Peel off the protective paper sheet. The patch will be sticky, so you don't need to use any adhesive spray or heat resistant tape. 330 degrees for 40 seconds - press from the front side of the patch.
 
HOUSEHOLD IRON: Let the patch rest for at least 1 hour after it's been sublimated. Peel off the protective paper sheet. The patch will be sticky, so you don't need to use any adhesive spray or heat resistant tape. 300 degrees (wool setting) for 30 seconds - press from the backside of the fabric. 
 
Patches can be dry cleaned or washed in a washing machine. Cold water, air dry.
